Procedure:

1. This form must be filled out by a Parent/Guardian

2. Re-entry cutoff time is 3:45 PM (I will not let anyone in after 3:45)

3. Student will group text Mr. Deegan and their parent upon arrival to the designated reentry door.  

4.  If Mr. Deegan does not respond to the text withing two minutes, the student athlete should call Mr. Deegan.  

5. Mr. Deegan will let them back in (only if this form is already filled out and the plan is in place).  

6. Anyone gaining access to the building to get to practice late without documentation (this form or the excused lateness form) will be escorted out of the building and dismissed for the day.  

7. Repeat offenders will be banned from participating in afterschool athletics.  

8. Mr. Deegan will create the group text once this form is filled out.
